Will I need a physician's order?
You do not always need a referring physician's order or referral for treatment at the Pain Center, although we work in collaboration with your existing physician to provide the most comprehensive care. To find a primary care physician, visit Swedish Covenant Hospital's Find a Physician online service or call the Call Center at 773-878-6888.

Will my insurance cover pain management therapy?
Pain management therapy is offered on an outpatient basis. Treatment is covered by most insurance policies, but check with your carrier to be sure.

When will I be able to get an appointment?
Wait time to see our physicians varies from 3-10 days, based on their schedule. We will do everything possible to accommodate your scheduling needs as quickly as possible.

We are open Monday - Friday, 9 a.m. - 5 p.m. and every other Saturday for several hours. Please call to check availability.

In order to best prepare for your appointment, please review the following important information.

What should I bring with me?
• Complete the Initial Pain Questionnaire before your appointment, if possible. Completing this in advance will make your appointment more efficient, and may allow you more time with your physician. You may also complete it on the day of your appointment.
• Bring a list of all medications you are presently taking, including doses and times, as well as the names of physicians prescribing.
• Bring any information that you may have, such as CT scans, x-rays or MRI reports.
• Bring your current insurance information.

How long will the appointment take? What will I be doing?
Pain treatment is very complex, and we want to do a very thorough assessment in order to develop an individualized treatment plan for you. Therefore, please allow 2-1/2 hours for your initial appointment. We ask that you complete the Initial Pain Questionnaire prior to your appointment to expedite the process.
Your appointment includes reviewing the questionnaire, your doctor's examination, a possible procedure and education about pain management. Your treatment plan will be discussed with you, and a report will be sent to your primary care physician.

Where do I go?
Please see the campus map and enter the Pain Center through the south entrance of the Galter LifeCenter (off the Parking Lot), under the sign "Boenhoeft Rehabilitation Center."

What if I need an interpreter?
Interpreters for Russian, Spanish and Korean are available. Please let us know if you need one, as they must be scheduled in advance. If you prefer, you can have someone come with you to translate.

What if I need to reschedule my appointment?
Please keep your appointment if possible. If you need to change your appointment, please call us at least 48 hours in advance.

We use benchmarks, like the performance outcomes below, to measure the impact and effectiveness of the Swedish Covenant Hospital Pain Center on your health. These measures may also give you an idea of the kinds of outcomes tha many of our patients experience at the Pain Center.

Within three visits to the Pain Center, 75% of our patients report an average of:
• 57% decrease in pain
• 58% increase in home activity
• 48% increase in active physical activity
• 57% improvement in self-care activity
• 54% improvement in sleep disturbed due to pain

The following results were obtained from 163 patients from 2006 to July 2007. In the following measurements, 10 is rated as "total disability" and 0 as "no disability." These outcomes are measured based on the Patient Disability Scale (PDS-Lofland 1998) in the following areas: pain scale, home activities, active physical activities, self care and sleep.

Pain Scale
In three visits, 42% of patients reported a decrease in pain from an 8 to a 5.
The pain scale is a universally used ranking system for pain. The scale describes the intensity of your pain from 0 (no pain) to 10 (worst possible pain).

Home Activities
In three visits, 47% of patients reported a decrease in disability when performing home activities from a 7 to a 4.

Active Physical Activity
In three visits, 43% of patients reported a decrease in disability when doing active physical activity from a 9 to a 5.

In three visits, 50% of patients reported a decrease in disability when doing self-care from a 4 to a 2.
