Pharmacy HFAP

Swedish Covenant Health’s Pharmacy was designated an Advanced Compounding Pharmacy with Distinction by HFAP, the nation’s original independent, accreditation program. This certification confirms that the Swedish Covenant Health Pharmacy is providing high quality care as determined by an independent, external process of evaluation.

“Swedish Covenant Health’s Pharmacy clearly demonstrates a commitment to quality and patient safety,” said Meg Gravesmill, CEO of AAHHS, HFAP’s non-profit parent company. “We base our decision on the findings of an extensive and thorough onsite review of the pharmacy against standards for organizational operations and clinical services that drive patient safety and continuous quality improvement. Swedish Covenant Health’s Pharmacy has earned the distinction of HFAP certification through its performance in successfully meeting those standards.”

“We’re proud to achieve this prestigious certification and be the first hospital pharmacy to be certified as an Advanced Compounding Pharmacy with Distinction,” said Alicia Juska, Director of Pharmacy Services at Swedish Covenant Health. “Earning HFAP certification is a significant achievement that recognizes our commitment to providing outstanding care to our patients and our community which is our top priority.”

The Swedish Covenant Health Pharmacy has also been recognized for its pharmacy and therapeutics-approved collaborative drug protocols and strong clinical presence in which pharmacists work closely with physicians and nurses, rounding daily on patients; its antibiotic stewardship program which promotes appropriate use of antibiotics while decreasing risk for toxicity, resistance, and unnecessary cost; its pain stewardship program which promotes appropriate use of opioids by balancing adequate pain control with decreasing the risk of adverse events, preventing addiction, and conserving limited opioid resources; and its pharmacy residency program which prepares future leaders in clinical pharmacy practice.
