The first few days and weeks home with a newborn can be a challenging time for everyone. Let Family Connects Chicago at Swedish Hospital lend a helping hand. We are proud  to partner with the Chicago Department of Public Health and Family Connects International to provide in-home support, guidance and connections to additional resources from Swedish Hospital nurses—at no additional cost to you if you live in Chicago. 

FREE at-home family and newborn care for families who give birth at Swedish Hospital and live in Chicago! 

Scheduling Your Personalized Visit

Family Connects Chicago services are offered through in-home or virtual/phone visits  to Chicago families who deliver at Swedish Hospital (at this time, families who live  outside of Chicago are not eligible). A Family Connects representative will call you within  the first few weeks after your hospital discharge to schedule this visit.
